New Hires/Transfers Benefits Checklist

If you started with COMPANY NAME before November 30, 2025, you will see two enrollment windows open when you go to enroll at www.Benefits.com:

1. One window for December 2025 benefits – effective 12/1/25
2. One window for 2026 benefits – effective 1/1/26

You have 30 days from your employment start date to complete your enrollment(s). Coverage will be effective retroactively to the dates listed above.

If you do not complete either enrollment, you will not have health benefit coverage for 2025 or 2026.
If you complete only the first enrollment (for 2025), your health benefit coverage that you elected for 2025 will be automatically rolled forward to the closest equivalent plan(s) for 2026.

Step 1: Review Your Choices

• Review your 2025 benefits guide (refer to the buttons on the right)

Step 2: Get Enrolled

• Collect your information:
  • PeopleSoft ID
  • Date of birth, Social Security Numbers and addresses for any benefit-eligible dependents that you are enrolling in coverage
  • Dependent verification documents must be submitted within your 30 day enrollment window. If documentation is not submitted, your dependents cannot be enrolled in coverage.

• Enroll in benefits:
  • Visit the Benefits Enrollment Portal (see instructions below) OR
  • Enroll via phone at 123.123.1234 (8:30 am – 7 pm ET, M–F)

• Review your elections:
  • Carefully review your elections to ensure you have selected the correct plans and enrolled all dependents
  • Save a copy of the benefit confirmation statement for your records​

Step 3: Receive ID Cards

• ID cards cards should arrive via US Mail within 2–3 weeks of enrollment

Step 5: Make an Appointment

• Call BHealthy Care Navigation to find providers and book appointments for well visits, screenings, and more! Our navigation team is available Monday through Friday, 8am to 7pm at 844.424.2628.

Employees who are covered by a collective bargaining agreement should check with their local human resources teams or union leadership to understand how these benefits may differ for them, if applicable.
